New Patient Forms

When you schedule your appointment, we will have you come in a few minutes early to complete the initial paperwork. If you prefer, you may want to download the forms from our website and complete them prior to your appointment.  This will speed up your first visit and reduce your wait time.

Dr. Robben will then meet with you to review your history and then perform a series of both orthopedic and neurological tests. She will be looking to see if spinal misalignments and nervous system interference are affecting your health and is the root of your current chief complaint. From this exam, Dr. Robben will determine if x-rays are necessary before chiropractic treatment.  Upon completion of the exam, Dr. Robben may begin your care, depending on your specific condition. Dr. Robben will not begin your care until she is confident that it is both safe and it would be of benefit to you.

After the first treatment, Dr. Robben will recommend a future treatment plan with 3 goals in mind:  relief of your current negative symptoms,  improved function of your spine and nervous system, and optimal health of your entire body.
